Those being treated for high blood pressure will benefit from a new program from the Heart and Stroke Foundation of Ontario.
The New Vision Family Health Team on Greenbrook Drive in Kitchener was one of 11 primary care sites across the province involved in the three-year demonstration phase for The Heart and Stroke Hypertension Management Program.
A doctor, nurse practitioner, dietician and pharmacist take a team approach in treating a patients’ high blood pressure.
Dr. Mel Cescon is with the New Vision Family Health Team.
He tells 570 News this approach empowers the patient to become responsible and take charge of their own blood pressure.
Dr. Cescon says they are also able to diagnose a significant percentage of people who were walking around undiagnosed.
More than 3,600 patients participated in the Canadian study which resulted in 41 per cent more of them keeping their high blood pressure under control.
