Documents compromised

The region’s public health unit says some confidential immunization forms were disposed of inappropriately last month.

Officials say the forms were submitted by some parents for students enrolled in junior kindergarten, senior kindergarten, Grade 1, high school, and child care centres in the 2009-2010 school year.

The forms include classified information about the students and were incorrectly placed in recycling bins instead of being destroyed.

A letter of notification will be sent to parents who may have been affected.
